Coventry Vaccination Clinic For Kids 16+

Eastern Highlands Health District has secured an arrangement with UConn Health to administer the Pfizer vaccine to kids 16 years or older at a school located closed clinics.

A consent to treat form will be needed for each 16 and 17 year old to have in hand at time of vaccination.  18+ students can sign at the vaccine site.
